Thyroid nodule, transoral surgery without a neck scar

volumineux nodule thyroidien chirurgie endo buccale sans cicatrice cervicale 1

A 36-year-old Black female patient with a history of keloid scars on many parts of the body presents with a large 4 cm right isthmo-lobar nodule deforming the neck. This nodule will be dissected via the transoral approach without a neck scar (Trans Oral Endoscopic Thyroidectomy Vestibular Anterior, TOETVA). Extraction of the very large nodule will be performed through a transaxillary trocar, at the cost of an additional small 1.5 cm scar, which will be hidden. In this case, extracting the nodule without fragmentation was easier than via the anterior vestibular approach, because of the prominence of the chin, which limits the space for its exit.
